Form Field #option371, 573, 556, 557 Issue

Hi! I received messages for the following errors :

1. Form Field #option371 Issue
2. Form Field #option573 Issue
3. Form Field #option556 Issue
4. Form Field #option557 Issue

I'm not sure what it is, how do I correct the errors. Please advice. TQ
