Problem with orders

When people order from any page that has multiple choices on it, and they place an order, I do not receive which item was ordered that is to be shipped.  I have an example.

This person ordered 12 cases of Apollo.  How am I supposed to know this?  What am I doing wrong?

I took a quick look at your site Bulk Orders Category.   I believe you do not have your options set up correctly.
