Order Form - Database Lookup

I have a need for the ability for the user to type in an ID number and have the order form populate information from a table in the database. My use case is that a limited number of users enter orders for a large number of dealers. I need to have the user identify the dealer and then have the dealer name and address appear in fields that can be edited if necessary. Thoughts?
