Hi, I thought I saw where to require customer login to access this form?  I was unable to find that again or anyway to prevent spammers from using that form repeatedly.  Please advise the resource I missed.

on website admin dashboard, you need to activate reCAPTCHA. Go to : System > Settings > General and paste the required keys which reCAPTCHA website will give you.

your ContactUsfrm will be working fine.
