Extension Support / VAT Tax ID extension removed our logo

Installed and working but it has removed our logo from the invoice page.
which template does it use now or how can we re-integrate our logo again.

Deleted the currencies and re-entered them one by one. Three will work without error but four gives an error with GBP

I Have AUD, EUR, USD and GBP as currencies and it is giving values for two or three at different times. so one or the other has a 0 value. Its always different even when I have only the AUD currency enabled I still get the error. How can I stop this.
