After the manual upgrade, clear all caches and check again. This should restore the missing form fields.

I don't see error messages about the installation, the only errors I see are these when attempting a check out. Is there a way to manually repair?

You might want to try running the 1.4.3 upgrade manually to resolve the issue

If you're currently on version 1.4.2, you can safely upgrade directly to 1.4.3.

A lot can happen during several consecutive upgrades, and the missing form errors (AddressFrm) indicate that the last upgrade to 1.4.3 was not completed correctly. These forms were introduced in 1.4.3, so if they're missing, some files or database changes likely didn't apply.

Please check your error log for any upgrade-related errors and post them here (you can create a .txt file and attach it).

Version 1.3.4 is quite outdated. It is recommended to make a full backup of your database and files, then update to the latest AbanteCart release. This should resolve the issue with the UPS extension and ensure you get all recent fixes and improvements.
