Hi Guys
Not sure where to post this one, but when I only have 1 Shipping and 1 Payment option installed on my Store, no Shipping, no Payment and no Shipping or Store Addresses are shown on the Order, nor on the E-mail.
If I add more Shipping and Payment options, all is fine. For this particular Store, I only use the Flat Rate Shipping and Bank Transfer Payment option. Using only 1 of each, causes the system to place the Order without showing the above mentioned info.
The other glitch I found, if I don't click Apply next to the Phone Number and click Place Order, the Order does not get placed and returns to the Landing Page. Phone Number Required at Checkout is disabled though.
I'm using Version 1.4.1
Any ideas?

what common restrictions was set for payment and shipping methods?
Did you tried to reset settings for both?

Even if I use Defaults, it does the same. I even tried it with Local Delivery too. Same thing, but the moment I enable more than one Shipping Method and more than one Payment method, it works fine.
1 Payment method + 1 Shipping Method = Problem
2 Payment Methods + 2 Shipping Methods = No Issue

Please give us more details. What shipping methods are enabled and post screenshot with your settings of them.
Also what payment method do you have enabled.
Also please post some test address of customer.
