A lot can happen during several consecutive upgrades, and the missing form errors (AddressFrm) indicate that the last upgrade to 1.4.3 was not completed correctly. These forms were introduced in 1.4.3, so if they're missing, some files or database changes likely didn't apply.
Please check your error log for any upgrade-related errors and post them here (you can create a .txt file and attach it).
I don't see error messages about the installation, the only errors I see are these when attempting a check out. Is there a way to manually repair?

You might want to try running the 1.4.3 upgrade manually (https://abantecart.atlassian.net/wiki/spaces/AD/pages/5275676/Upgrade+manually) to resolve the issue
