For some reason, the customer info is not being populated on order invoices. Even the old invoices before the upgrade are missing the customer info. The format is displaying as:
{firstname} {lastname}
{company}
{address_1}
{address_2}
{city}, {zone} {postcode}
{country}
I removed the {company} field from the USA Address format under System->Localization->Countries but it is still showing.

Do you have any errors in the AbanteCart error log after upgrade? Possibly not all files were upgraded.
For the format change it should affect only new invoices
Nothing in the error log.
It was the Mandatory Telephone Option that caused the problem.
Was the issue solved by installing the extension version that was compatible for your Abantecart 1.2.6?
