Possibly you have only partial update. Try to clear AbanteCart cache if issue not resolved try manual update (backup first!)

Make sure Forms Manager extension is installed and enabled (status ON)

you can reinstall it without any affects on your data. Just uninstall and install.

Where i can download this extension Forms manager?? On market is only Custom form manager for $45.
