Contact us form not displaying

Hi Members,
I need an ****** help  I have created one more form(from form manager) as per requirement but to try it how it looks In my layout I used the default contact us form

now when I am clicking the original contact us form that I had earlier from my content the form is not displaying just the content is displayed?
P.S: My form is stillther ein form manager and is ON
IS there a setting that I misselected? my original contact us Form is turned on in form manager and It does have content in it
IS the a page layout(content area ) of contact us form
In my case ony the adderess and telephone number is display the form doesn't show up

Thanks, I figured what my problem was I have entered wrong form name in the input field but now it works thanks again for your help
