Add a Form to a new page

Is it possible to add a contact form to a newly created page for example sitename.com/contact-form

I have created a new form using the form manager and have created a new page called contact us.

How would i get that form to show on the newly created page.

You need to create a Form Block (Design > Blocks) and then add it to page layout

Ive created the block but the page I want it to appear on doesn't appear in the layout and block manager. 

I need to add the from to a page called contact us.
