How do you add a link to a new custom form similar to contact page

Same question as OP.

I have created a custom form and I want it to function the same as the contact us form. Eg; customer clicks on "contact us" in the footer and it takes them to the contact us form where they fill in their details etc. How would I achieve this with my own custom form?

I can't achieve this by adding the custom form to a layout as I want it to open its own separate page.

Any help would be appreciated, thanks.
